Arterial hypoxemia is a condition in which there is a decrease in arterial oxygen levels. In individuals with COPD, which includes emphysema, this occurs due to several factors that interact in a complex way. One of the main factors is the imbalance between alveolar ventilation and veno-arterial shunting, as well as the distribution of alveolar ventilation and pulmonary capillary perfusion. This mismatch leads to ventilation-perfusion (V/Q) mismatch, which means that air flows to parts of the lungs that have no blood flow, and blood flows to parts of the lungs that have no air flow.
